This notice explains the information QR Safety Scan collects for the MVP pilot reporting workflow and how it is used.

Information submitted in reports
QR Safety Scan collects the report details entered on the public reporting form, including category, description, optional contact information, optional photos, QR code context, submission time, IP address, and user agent for spam and operational review.
How report information is used
Report information is used by the customer company to review workplace safety concerns, document corrective actions, track status, and export safety response records.
Anonymous reporting
When anonymous reporting is enabled and selected, contact details are not stored with the report. Report content, photos, QR context, IP address, and user agent may still be stored for safety workflow and abuse prevention.
Dashboard accounts
Company admins, managers, and QR Safety Scan super admins use authenticated dashboard accounts. Dashboard access is limited by role and company membership.
Data access and storage
Customer report data is separated by company. Uploaded photos and logos are stored in private storage, and dashboard viewing uses authorized server-side access and signed URLs where appropriate.
Legal review
This MVP privacy notice is working product copy for pilot readiness and should be reviewed by appropriate legal counsel before broader commercial launch.
